- Compare the MHSMHP CarePoint registries that maybe useful for case managers.
- Discuss at least one reason why the Health Services Registry is more specific in case finding that other tools in the MHSPHP database.
- Relate use of registries to case finding and management of cases.
- Summarize the purpose of the Disease Management Program.
- Identify patients that would benefit from Disease Management services.
- Examine how Disease Management aligns with the Quadruple Aim.
- Explain how an integrated medical management program supports population health.
- Outline least two goals of an integrated medical management program.
- Breakdown the three modalities utilized under an integrated medical management program and where to apply them.
- Differentiate between Healthcare Effectiveness Data and Information Set (HEDIS) metrics and Clinical Practice Guidelines (CPG’s).
- Evaluate Defense Health Agency (DHA) focused HEDIS measures and how these translate into quality patient care.
- Know where to locate information regarding the methodology of each measurement in CarePoint MHSPHP.
