Learning Objectives

  1. List at least two applications that can be of use to medical management staff.
  2. Compare the MHSMHP CarePoint registries that maybe useful for case managers.
  3. Discuss at least one reason why the Health Services Registry is more specific in case finding that other tools in the MHSPHP database.
  4. Relate use of registries to case finding and management of cases.
  5. Summarize the purpose of the Disease Management Program.
  6. Identify patients that would benefit from Disease Management services.
  7. Examine how Disease Management aligns with the Quadruple Aim.
  8. Explain how an integrated medical management program supports population health.
  9. Outline least two goals of an integrated medical management program.
  10. Breakdown the three modalities utilized under an integrated medical management program and where to apply them.
  11. Differentiate between Healthcare Effectiveness Data and Information Set (HEDIS) metrics and Clinical Practice Guidelines (CPG’s).
  12. Evaluate Defense Health Agency (DHA) focused HEDIS measures and how these translate into quality patient care.
  13. Know where to locate information regarding the methodology of each measurement in CarePoint MHSPHP.
