Day 3: Forensic Healthcare Program, Patient Self-Management Goals, Care Plans, Clinical handoff to the VA / Transitions of care

Target Audience

This activity provides continuing education for nurses, social workers, and case managers across the Military Health System (MHS). A certificate of attendance is available for other attendees.

Learning Objectives

  1. Analyze current Department of War policies and procedures established to support patients seeking care associated with sexual trauma.
  2. Differentiate between the clinical components of a forensic healthcare examination and the legal protocols of evidence collection.
  3. Outline follow-on medical care pathways for patients of sexual trauma and the multi-disciplinary role of the healthcare provider.
  4. Evaluate the role of case management involvement in coordinating longitudinal care for patients with a history of interpersonal violence.
  5. Define self-management and self-management support.
  6. Describe the Joint Commission requirements and survey expectations for self-management goals.
  7. Identify techniques and strategies to develop, document, and sustain patient-centered self-management goals.
  8. Describe the purpose and key components of case management nursing care plans in promoting continuity of care and patient safety.
  9. Explain how individualized, evidence-based care plans support patient-centered care and address each patient's unique needs.
  10. Discuss the role of comprehensive care planning in improving communication and collaboration among the interdisciplinary healthcare team.
  11. Identify strategies to engage patients and families in the care planning process through shared decision-making and education.
  12. Apply principles of effective case management to develop comprehensive, patient-centered nursing care plans that support quality outcomes.
  13. Summarize the role of a VA Liaison and the VA Liaison Program.
  14. Be familiar with VA resources to assist transitioning service members.
  15. Knowledgeable on the VA referral process.
