ForwardDay 3: Forensic Healthcare Program, Patient Self-Management Goals, Care Plans: Development & Best Practices, Clinical Handoff to the VA / Transitions of Care
Target Audience
This activity provides continuing education for nurses, social workers, and case managers across the Military Health System (MHS). A certificate of attendance is available for other attendees.
Learning Objectives
- Analyze current Department of War policies and procedures established to support patients seeking care associated with sexual trauma.
- Differentiate between the clinical components of a forensic healthcare examination and the legal protocols of evidence collection.
- Outline follow-on medical care pathways for patients of sexual trauma and the multi-disciplinary role of the healthcare provider.
- Evaluate the role of case management involvement in coordinating longitudinal care for patients with a history of interpersonal violence.
- Define self-management and self-management support.
- Describe the Joint Commission requirements and survey expectations for self-management goals.
- Identify techniques and strategies to develop, document, and sustain patient-centered self-management goals.
- Describe the patient-activated outpatient care-planning process from comprehensive assessment through evaluation and revision.
- Differentiate medical diagnoses from NANDA-I nursing diagnoses and select current 2024–2026 terminology based on the patient’s human response.
- Develop SMART goals that incorporate the relevant baseline, one measurable outcome, a defined target, timeframe, and evaluation criteria.
- Distinguish Patient-Activated Actions from professional case-management interventions.
- Use objective outcome measures to evaluate progress and determine whether the goal is met or revision is required.
- Summarize the role and mission of the VA Liaison and the VA Liaison Program.
- Identify key VA programs, benefits, and support resources available to assist transitioning Service Members.
- Outline the VA referral process and required timelines for active-duty Service Members transitioning care.
