Program Description

This activity is a part of the Womack Army Medical Center Pharmacy Monthly Trainings. This iteration will review pharmacist interventions during transitions of care.

Target Audience

This activity provides continuing education for pharmacists and pharmacy technicians. A certificate of attendance is available for other attendees.

Learning Objectives

  1. Comprehend transitions of care.
  2. Recognize how pharmacists play a role in transitions to care.
  3. Identify when errors/breaks can occur during transitions of care and devise ways to mitigate these errors.
