Primary Care Medical Home (PCMH) Executive Skills Leadership Seminar (PELS)
Target Audience
This activity provides continuing education for physicians, nurses, physician assistants, pharmacists, pharmacy technicians, psychologists, dentists, dental hygienists, dental technicians, and healthcare executives.
Learning Objectives
- Relate the importance of the 2017 NDAA to the MHS transformation.
- Explain the common challenges associated with PCMH clinics.
- Discuss mechanisms to improve communication such as Secure Messaging, TRICARE Online, MHS GENESIS Patient Portal and Nurse Advice Line.
- Summarize which tools and resources are available for population health.
- Compare the roles of each MM team member and how they interact and enhance the PCMH team.
- Describe empanelment requirements and which policies to use.
- Review methods to validate compliance.
- Outline the Leader’s role in clinical quality improvement, High-Reliability Organizations (HRO) and how to lead the change.
- Identify which three strategic initiatives that Market and MTF plans will align to.
- Devise strategies to implement within the MTF to improve metrics.
- Optimize the skillsets of the rest of the MTF staff to provide care to the PCMH patient.
- Recognize the typical barriers to effective and efficient PCMH team operations.
- Integrate all prior lecture information into the improving scenario MTF performance.
