Target Audience

- Describe difference in diabetes treatment guidelines.
- Describe the importance of performing a medication reconciliation.
- Explain standardized documentation of medications in the electronic health record system.
- Identify the MEDCOM metrics.
- Explain reasons and indications for tapering opioid regimens.
- Identify opioid reduction strategies that can be implemented by clinical pharmacy.
- Describe the role of the clinical pharmacist in both preconception and contraception care.
