Program Description
The Diabetes Champion Course provides training for primary care teams (provider & support staff) to assess and improve the diabetes care provided at their respective MTF. Focus is on increasing individual knowledge, assessing clinical processes, facilitating team dynamics, and developing and implementing quality improvement initiatives.
Target Audience
This CE/CME activity is accredited by the DHA, J-7, Continuing Education Program Office (CEPO). This activity provides continuing education for physicians, nurses, physician assistants, pharmacists, pharmacy technicians, registered dieticians, and dietetic technicians. A certificate of attendance is available for other attendees.
Learning Objectives
- Discuss the scope of diabetes.
- Distinguish strategies to optimize provider and team behavior.
- Identify risk factors and metabolic defects in diabetes and pre diabetes.
- Summarize the clinical practice guidelines from the VADOD and American Diabetes Association.
- List the indication for Continuous Glucose Monitoring.
- State factors to consider when prescribing medications to treat diabetes and determine proper manage
- Outline programs at DCOE for patient and provider educational opportunities.
- Discuss recognition and treatment of hypoglycemia.
- Analyze the strategies for dosing and titrating insulin.
- Summarize CDC National Diabetes Prevention Program (NDPP).
- Define classes oral/injectable medications, explain use to treat type 2 diabetes and assess effectiveness.
- Apply VA/DOD sequential approach plan medical management of diabetes.
- Develop strategies for initiation/titration of insulin and/or non-insulin therapy.
- Recognize elements in clinical case study to construct treatment plan.
- Develop an action plan.
- Explain the importance of preconception counseling for women with diabetes of childbearing age.
- Identify the diabetes/hypertensive/ cholesterol medications contraindicated during pregnancy.
- Analyze the health care team workflow, roles and actions in the management of a PWD
- Compare strategies to facilitate behavior change.
- Outline strategies for providing Diabetes Self-Management Education and Support (DSMES) to PWD.
- Identify ways to reduce risk of hypoglycemia during exercise.
- Describe Motivational Interviewing (MI) and its relation to Shared Decision Making (SDM).
- Summarize changes in medication regimens for known surgical or diagnostic procedures.
- Evaluate a pediatric patient with overweight/obesity for secondary causes and co morbidities.
- Summarize resources to provide support to the multidisciplinary team.
