Target Audience

This activity provides continuing education for pharmacists and pharmacy technicians across the Military Health System (MHS). A certificate of attendance is available for other attendees.

Learning Objectives

  1. Explore heparin PowerPlan and standardization.
  2. Distinguish which patients require warfarin over direct-acting oral anticoagulant (DOACs).
  3. Generate a treatment plan for outpatient management of venous thromboembolism (VTE) (initiation, follow up, duration).
  4. Describe appropriate transition to and from warfarin.
  5. Discuss low/high weight DOAC use.
  6. Discuss when/how long to hold anticoagulation prior to procedures.
  7. Discuss factors dictating the restart of anticoagulation after procedure and/or bleeding.
  8. Describe the SPARCtool, MAQI2, MHSG AC Workflow, and RapidRecap tools.
  9. Explain the typical length of therapy based on diagnosis.
  10. Discuss follow up intervals for DOAC therapy.
  11. Describe Heavy Menstrual Bleeding ACForum 2026 updates.
