ForwardComprehensive Systematic Analysis Training OCT 2026
This instructor-led, virtual course consists of engaging learning modules and mentoring and networking opportunities. The Department of Defense Patient Safety Program designed this practical, actionable curriculum for new/entry-level Patient Safety Managers and other Patient Safety Professionals in military treatment facilities who have been in their role for fewer than twelve months. This course provides students with an overview of the policy, process, and deliverables of Comprehensive Systematic Analysis, focusing on Healthcare Safety Review.

Target Audience
This activity provides continuing education for physicians, nurses, physician associates/physician assistants, pharmacists, pharmacy technicians, dentists, dental hygienists, dental technicians, and healthcare executives across the Military Health System (MHS). A certificate of attendance is available for other attendees
Learning Objectives
- Identify Comprehensive Systematic Analysis (CSA) Types used by the Defense Health Agency.
- Explain the uses of the Human Factors Analysis and Classification System (HFACS) categorization system to report causes of patient harm.
- Apply HFACS analysis to case example in a large group setting using Vevox.
- Describe Restorative Justice.
- Review and Apply Just Culture Algorithm to clinical scenarios.
- Discuss actions to ensure safety and begin evidence for CSA.
- Describe leadership actions to support staff after a Patient Safety Event (PSE).
- Identify critical touchpoints in a harm event to include in the event sequence.
- Describe the importance of thorough Factor identification and reporting to developing effective Corrective Actions.
- Identify common errors in Investigation Models.
- Explain the criticality of a linear relationship between causal factors, root causes, and Corrective Actions.
- Describe challenges in using a team to describe system error in a way that expresses group consensus.
- Discuss the process for selecting the appropriate databases and evidence to answer a clinical question.
- Explain the role of stakeholder engagement in Corrective Action implementation and sustainment.
- Describe requirements of the Health Safety Report (HSR) template.
- Apply the concepts of scoring using Feasibility, Acceptability, Cost/Benefit, Effectiveness, Sustainability, Risk (FACES-R).

DHA J-7 CEPO has been authorized by the American Academy of Physician Associates (AAPA) to award AAPA Category 1 CME credit for activities planned in accordance with AAPA CME Criteria. This activity is designated for 14.25 hours of AAPA Category 1 CME credits. Approval is valid until Thursday, November 12, 2026 - 11:59pm ET. PAs should only claim credit commensurate with the extent of their participation.
