ForwardDay 5: Referral Management, SHCP / ECHO, VA Polytrauma / DOD-MOA, Federal Recovery Consultant
Target Audience
This activity provides continuing education for nurses, social workers, and case managers across the Military Health System (MHS). A certificate of attendance is available for other attendees.
Learning Objectives
- Discuss applicable CFR and DHA Guidance governing referral management.
- Describe structure and responsibilities of Integrated Referral Management and Appointing (IRMAC).
- Identify Referral Management workflows within MHS GENESIS.
- Explain how to establish clear, localized IRMAC guidance.
- Discuss challenges and strategies to optimize referrals and appointing.
- Identify key performance measures and available reporting tools.
- Recognize community connection and collaboration resources.
- Summarize the Comparable to Extended Care Health Option (ECHO) benefit for Active Duty Service Members (ADSMs).
- Identify services and/or supplies authorized under the benefit.
- Determine which ADSMs meet criteria for services and/or supplies.
- Explain the purpose of a Supplemental Healthcare Program Waiver (SHCP).
- Recognize Statutory/Regulatory Language, Non-waiverable Services, and SHCP Waiver Instructions.
- Describe the Veteran Affairs (VA. Polytrauma System of Care inpatient programs and resources available.
- Identify point of contacts for the Polytrauma System of Care and TRICARE/Defense Health Agency (DHA. for care coordination
- Recognize three best practices for the treatment and care coordination of brain injury, polytrauma injury and their sequela.
- Gain insight on the historical background of the MOA.
- Explain the patient population eligible to be admitted under a MOA authorization.
- Identify the distinct roles of the MTF, VAMC, and DHA-GL staff members and understand the importance of communication in the co-management of ADSMs in the MOA.
- Apply the MOA authorization process when referring cases into the VAMC and/or requesting continued stay requests.
