Target Audience

This activity provides continuing education for nurses, social workers, and case managers across the Military Health System (MHS). A certificate of attendance is available for other attendees.

Learning Objectives

  1. Diagnose a case situation before acting - matching the response to the type of problem.
  2. Apply targeted interventions for the post-discharge gap, the unowned team, surge/crisis, and idea sustainment.
