Program Description: 

This instructor-led, virtual course consists of engaging learning modules and mentoring and networking opportunities. The Department of Defense Patient Safety Program designed this practical, actionable curriculum for new/entry-level Patient Safety Managers and other Patient Safety Professionals in military treatment facilities who have been in their role for fewer than twelve months. This course provides students with an overview of the policy, process, and deliverables of Comprehensive Systematic Analysis (CSA), focusing on Healthcare Safety Review (HSR).

Target Audience

This activity provides continuing education for physicians, nurses, physician assistants, pharmacists, pharmacy technicians, dentists, dental hygienists, and dental technicians. A certificate of attendance is available for other attendees.

Learning Objectives

  1. Identify Comprehensive Systematic Analysis (CSA) Types used by the Defense Health Agency (DHA).
  2. Describe the structure and organization of the Human Factors Analysis and Classification System (HFACS).
  3. Describe leadership actions to support staff after a Patient Safety Event (PSE).
  4. Identify requirements of the Health Safety Report (HSR) template.
  5. Evaluate actions in supporting Second Victims and Restoration.
  6. Identify what initial actions you would take to manage a PSE.
  7. Identify critical touchpoints in a harm event to include in the event sequence.
  8. Describe how to use the Elements of Analysis to ensure a thorough and credible investigation.
  9. Identify common errors in Investigation Models.
  10. Explain the criticality of a linear relationship between causal factors, roots causes, and corrective actions.
  11. Develop a Factor that meets Deficiency/Effect/Impact framing.
  12. Discuss key influences for Corrective Action implementation.
  13. Explain the role of stakeholder engagement in Corrective Action implementation and sustainment.
  14. Develop Corrective Action(s) to address a selected Factor identified during the review.
  15. Assign a Feasibility, Acceptability, Cost/Benefit, Effectiveness, Sustainability, Risk (FACES-R) score.
